Abstract: Background The regions have a strong focus on increasing the value creation in the health care sector. The initiation of projects to optimize the patient’s pathway through the Health Care system is an example of this focus. The Respiratory Center East, at Glostrup Hospital, is a highly specialized section, where such patient’s pathway process optimizations are prioritized. The goal is to optimize the patient’s pathway through a system spanning several departments at multiple hospitals. Conclusion and perspective Patients are not only clients who want the best for themselves, they are also citizens who are part of the community and as community members they typically exhibit a great willingness to participate in the optimization processes. The professionals must be ready to incorporate the patients into their sphere in order to create value for society. Dissertation thesis statement examines How can patient care optimization in Respiratory Center East contribute to public value creation ‐ a case study How Respiratory Center East create public value ‐ From a leader ‐ a doctor and a patient perspective What inhibits the doctor’s ability to optimize patient care ‐ From a management perspective How to promote the medical cooperation in patient care optimizations ‐ From a medical perspective Perspective and Theory The constructivist perspective is chosen, as I am interested in investigating a process that cannot easily be separated from its context. Within the constructivist perspective I will be using the theory of public value, the theory of motivation and theory of leadership of the self‐managing employee. My managerial development is analysed by the aktant model. Method I have chosen a case study design, as this is suitable in studies where the interest is to analyse complex social phenomena’s. “Elite‐interview” is selected as a qualitative method within the constructivist perspective. As I am the interviewer in the “Elite‐interviews” within my department, my role as collector and analyser of empirical data will be analysed and discussed. Analysis I have analysed the three sub‐questions using the chosen theories and the key findings are:  How does Respiratory Center East create public value? ‐From a leader ‐ a doctor and a patient perspective Patient pathway optimization can help support creation of public value in Respiratory Centre East, when willingness patients are asked to participate in the optimization processes, not only as patients, but also as citizen actively pursuing what is best for the society. The theories put forward by Horner & Hutton, Benington and Moore (2011, chapter 6) support these findings.  What inhibits the doctor’s ability to optimize patient care? ‐ From a management perspective From the start there were no joint understanding among doctors for making patient pathway optimization, and this has been one of the main hindrances for designing the optimization pathways. Doctors have, during the last 4 years, had to accept the role as autonomous doctors, limited by leaders that apart from the medical work, had others priorities. This follow the theory put forward by Raastrup Kristensens (2011) about the autonomous employee, losing the autonomy due to factors other than the mere medical based.  How to promote the medical cooperation in patient care optimizations ‐ From a medical perspective Patient pathway optimization can support public value creation in The Respiratory Center East, at Glostrup Hospital, when collaboration between different medical teams is enhanced. This follows the theories of: Drucker (1999): When doctors utilize their autonomy to create new task; of; Boswell et al. (2006): By knowing the end goal of the collaboration and finally by Osterloh and Frey (2002): When the doctors as responsible for the out‐come, they are intrinsic motivated to do their best for the patients. The author’s leadership role is analysed via the aktant model at the end of the thesis.
